I've got a bigger and faster MSD card coming.

What do I need to do to it before I put it in the DHD?

Does it need a particular disk format?

My PC has a MSD reader. Should I copy the contents of the old one to the new one using my PC?

Copying the data over should be the easiest way. 
According to my PC, the Desire's MSD card is formatted in FAT32. 

I'd suggest :
1 ) copying the data from your old card to a folder on the PC
2 ) formatting the new card in FAT32
3 ) copying the data to the new card
4 ) trying the new card in the phone

I did it more or less the same way.

When I bought my 16GB Sandisk, it was already formatted.  I simply put it in the phone, put the old 4GB card (via an adapter) in the PC's USB port, connected the phone to the PC and copied the data straight over.  Worked like a charm.

Yeah. Nothing much to do, but for safety do format the card before you use it. FAT32 partition.
